You will need the following documents:
- Current Mortgage Statement
- Proof of identification
- Driver’s license or other government issued photo ID AND
- Copy of Social Security Card
- Proof of residency
- Current utility bill
- Proof of Ownership Off Reservation – Tribal Member must have an ownership interest
- Property Tax Bill OR
- Copy of most recent recorded deed – Enrolled Tribal Member Applicant must be on Deed
- Proof of Tribal Membership – Tribal Member must be Main Applicant
- CIB
- Income Documents – You must provide all documents for each of the following applicable types of income your household receives
- Reservation – Homesite Lease
- Home Repair Applicants must submit 3 repair quotes from a licensed trade professional/contractor
- Please note that additional documents may be requested
